Licensee: The person (or group of persons) you are giving the license to record songs from your catalog.
When signed by both parties this document will constitute a valid license for the purpose of manufacture and distribution of Phonorecords (including Compact Discs, any cassette configuration, and any other audio device) upon all terms and conditions set forth herein for licensee's use of the following musical composition (compositions).
Composition (compositions) Title (Titles), writer (Writer's) publisher, ©copyright date, performing rights organization, B.M.I.-A.S.C.A.P.-S.E.S.A.C. which is to be included on licensee's recording project.

1. Licensor hereby grants to the licensee the non-exclusive and non-assignable right to manufacture and distribute recordings embodying a single performance of the composition (compositions) as described below.

2. In connection with the right granted herein, licensee shall have the right to make a musical arrangement of the composition (compositions) to the extent necessary to conform to the style/interpretation of the performance involved, provided that any such arrangement shall not change the basic melody or fundamental character of the composition (compositions) Licensee further agrees that the title of the composition (compositions) will not be used as the title of the recorded project without express written consent from the licensor.
3. In consideration for the rights granted herein, licensee agrees to pay licensor the current statutory rate of *$0.0850 thru 12/31/04 for each phonorecord embodying the composition under licensee's authority. A copy of the recording shall accompany payment. This license is good for life of the recording project identified herein and any royalties due based on any re-orders shall be at the then-current statutory mechanical rate ($0.090 per copy 01/01/05 thru 12/31/07 and adjusted January 1st @ each two year interval thereafter) and is to be accompanied by a copy of this license. THIS FEE REPRESENTS THE 100% OWNERSHIP OF THE COMPOSITION (COMPOSITIONS) AND IS TO BE PAID TO (YOUR PUBLISHING COMPANY)
4. Licensor hereby warrants and represents that licensor is the lawful owner of the copyright in the written referenced composition (compositions) or is a legally authorized agent of the owner and possesses fall authority to grant the rights described herein.
5. This document constitutes the full agreement between the licensor and the licensee and shall be binding upon the heirs, legal representatives, successors and assigns of the parties to this agreement when signed. This document is printed in duplicate and when signed each shall serve as an original (One copy to LICENSOR and one copy to LICENSEE)
